RDASH Consultation on Care Programme Approach

RDASH asked us to share with you the patient and carer events they have planned as part of the Moving Towards Personalised Care project which is currently taking place in the Trust.
The aim of these events is to listen. If you are a patient, carer or family member who has had experience of CPA (Care Programme Approach) we want to hear what works well and less well while making changes to the way we deliver care across the services.
