Health Check Activity Pack

The North Yorkshire Learning Disability Partnership Board have been working with the CCG to put together an annual health check activity pack for people with a learning disability. This may be useful for families or carers in supporting and encouraging people to access their annual health checks. The pack contains information about what to expect when going for an annual health check, health action plans, information about medicines and where to go for support if you are feeling unwell.
