Have Your Say on Healthcare Transition Services
The Rotherham Hospital Foundation is asking families to share their experiences to help improve the transition process for young people moving through healthcare services.
They are keen to hear from everyone, even if you’ve had little or no direct experience with healthcare. Your feedback is still valuable in ensuring the service is inclusive and meets the needs of all young people who may access it.
All responses will be completely anonymous. The questionnaire will not collect your personal details, such as your name or email address, unless you choose to provide them.
We encourage our members to take part and help shape a service that works better for Rotherham young people and their families.
